Chronic venous insufficiency

Common, progressive, and often very treatable with the right care and guidance.

Chronic venous insufficiency (CVI) occurs when the one-way valves in your leg veins do not close properly, allowing blood to flow backward and pool in the legs. It’s more common than most people think, and it rarely gets better on its own.

How chronic venous insufficiency can progress

Doctors describe vein disease in stages, from C1 to C6. Without treatment, it often moves through them over the years.

  1. C1
    Calf with fine spider veins
Spider veinsTiny red, blue, or purple veins just under the skin.
  2. C2
Calf with bulging, twisted varicose veins
Varicose veinsBulging, twisted veins, often with aching or heavy legs.
  3. C3
    Lower legs with swollen ankles
Leg swellingPuffy ankles and lower legs, often worse by evening.
  4. C4
Lower leg with darkened, discolored skin near the ankle
Skin changesDarkened, itchy, or hardened skin near the ankle.
  5. C5–C6
Ankle with a venous leg ulcer
Leg ulcersSlow-healing sores near the ankle, healed (C5) or open (C6).

Without treatment, it can keep progressing.

Studies that followed people with vein disease over many years found:

Almost 1 in 3 people with varicose veins developed skin changes within 13 years.2

About 1 in 50 adults with varicose veins develops chronic venous insufficiency each year.1

Why treating it matters

  • Relief from aching, heaviness, and swellingClosing the faulty vein stops blood from pooling, so symptoms usually ease.
  • Leg ulcers heal fasterIn a 450-patient trial, treating the faulty vein early healed ulcers in a median of 56 days, versus 82 days when treatment was delayed.4
  • Ulcers are less likely to come backTreating the faulty vein cut ulcer recurrence at 12 months by more than half, compared with compression stockings alone.3
  • Little time away from your dayTreatment is done in our office, and most patients return to normal activity the same day.

Why it happens

Healthy Vein vs Incompetent Vein

Tiny one-way valves keep blood moving up toward your heart. When they weaken, blood slips back down, pools, and stretches the vein.

Healthy vein

Valves close behind each push.

With every step, your calf muscles help move blood upward toward your heart. One-way valves open to let blood pass, then close to prevent it from flowing backward.

Incompetent vein

Blood slips back and pools.

When vein valves do not close properly, blood can flow backward and pool in the leg. Over time, the extra pressure can cause the vein to enlarge, twist, and ache.

After treatment

Flow reroutes to healthy veins.

We seal the diseased vein, allowing blood to reroute through nearby healthy veins.

I had a very extreme case of bulging varicose veins.

After standing or being at work all day, there was actually some pain involved and discomfort.

I couldn’t wear shorts.

I just made an appointment. Just felt like it was a good decision.

When I came in, from the get-go, everybody was real professional. There was no stress involved.

It was real easy to talk and communicate with the doctor and all the staff. The overall experience was awesome.

It changed my life completely. I can put on a pair of shorts now.

I would highly, highly, highly recommend this place.

See all questions

Does vein treatment hurt?

Most patients describe treatment as very manageable. You may feel brief pressure or a pinch from numbing medicine, and some mild soreness afterward is possible. Treatments are done in the office with local anesthesia, not general anesthesia, and most people walk out on their own. We will explain what to expect before your procedure and make sure your questions are answered.

How do I know if I need treatment or if it’s just cosmetic?

Aching, heaviness, swelling, cramping, or skin changes point to a medical problem. A painless ultrasound shows whether your vein valves are leaking. Spider veins with no symptoms are usually cosmetic.

Will my insurance cover it?

When vein disease causes symptoms such as pain, heaviness, swelling, cramping, or skin changes, treatment may be considered medically necessary. We accept most major health insurance plans, but not Kaiser or UHA. We verify benefits before your visit and explain the next steps as clearly as we can. Coverage depends on your plan, symptoms, and ultrasound findings. More about insurance and cost.

How soon can I go back to work?

Most patients return to normal activity the same day. We recommend walking afterward and may ask you to wear compression stockings for a short time.

Do varicose veins come back after treatment?

A treated vein stays closed. New veins can develop over time, because vein disease can be hereditary. Follow-up visits help catch changes early.
